Graduate Certificate in Professional Studies in PVR

Application Deadline:
June 1

Please download, print, complete and mail the application form and supplemental application materials. Download the 2009 Application Packet.

Included in this packet are requests for the following: 

• Application form
• Residency Information form
• Supplemental PVR application form
• Transcripts form all previous institutions
• A brief 500 word essay stating why you would like to attend the PVR Certificate program.
• A joint etter from Agency Director and your Direct Supervisor which supports your application to the PVR program. The letter should include the following:
• Your agency's understanding that you will be applying to the PVR certificate program.
• A statement that your agency will be supportive of the student applying course knowledge and skills with people with psychiatric disabilities in the worksite. 
• An understanding that the agency may be in contact with PVR program faculty to aid in the evaluation of the student.
• Names and Signatures of the Agency Director and Direct Supervisor, with contact information to include phone number and email address.
  

Please send(by mail) all transcripts, and the employment information, an optional resume, essay, and agency's letter of support (signed by both Direct Supervisor and Agency Director) and the $60 application fee to:
